The following educational materials are compiled for use by medical professionals and clinicians. They include several sideline assessment, symptom assessment and other evaluation tools that may be beneficial to Physicians, Clinicians, Licensed Athletic Trainers, and other medical professionals and clinicians that work with athletes. Our hope is to build a library of educational materials with contributions from all collaborative members under the WIsportsconcussion.org banner. Athletes with a concussion are different than other injured athletes. They have decreased self-awareness, so they have difficulty answering open-ended questions and often will not accurately report symptoms unless they are asked yes-no questions. The following forms and tools can help improve the identification and diagnosis of concussion. The WSCC recommend that you choose the one(s) which best fits your practice both on the sideline and in the clinic or emergency department setting.
