Parental Consent is required in order for individuals under 18 years of age to take the ImPACT™ Baseline Test. The organization providing baseline testing to your child should provide you with a Parental Consent Form. In the event you have not received this form, please print out 3 copies of the form below, sign and return 2 of them to the organization that will be testing your child.
